ABSTRACT

OBJECTIVE:To analyze the research status and hotspot frontier of polypharmacy in the elderly in recent ten years , and to provide reference for polypharmacy study in the elderly in China. METHODS :The related literatures about polypharmacy in the elderly were retireved from Web of Science core database during Jan. 1st,2010-Dec. 31th,2019. CiteSpace 5.6.R5 software was used to analyze the quantity of pulished literatures ,authors,country/area,institution,co-citation of journals and literatures. The keywords were taken as nodes for co-occurrence analysis ,cluster analysis and Burst analysis ,the visual maps were drawn and the results were analyzed. RESULTS :A total of 3 521 literatures were included ,and the quantity of published literatures about polypharmacy in the elderly was on the rise ;high-yield authors mainly were Bell JS and Hilmer SN which were the representative with 36 papers published ,accounting for 1.02% of the total. America (929 literaturesm 26.3%)and University of Sydney (101 literatures,2.87%)were the most high-yield country and institution respectively ;Journal of the American Geriatrics Society was the journal with the most articles (2 202 pieces). The most frequently cited literature was those published by Radcliff S team in 2015 to update Beers standard to reduce potential inappropriate drug use ;the research on how to optimize the prescription for the elderly published by Spinewine A team in 2007 had the highest co-citation centrality. The hot keywords were elderly ,polypharmacy and risk factor ;keywords with high centrality included quality of life ,polypharmacy and Beers standard. Research hotspots included risk factor ,outcome index and reduction strategy for polypharmacy. Six clusters were formed ,including Breast cancer , Prescription,Disorder,Schizophrenia,Mediation attachment and Trial ;complications,pain,frailty,health-related quality , intellectual disability were at the forefront of research. CONCLUSIONS :In the past 10 years, the research on polypharmacy in the elderly has been paid more and more attention by scholars ;the United States and Australia have made great contributions in this field;the research mainly focused on the impact of polypharmacy on health outcomes and the strategies for reduce polypharmacy. In the future ,more attention will be paid to the related research on psychological ,cognitive and debilitating effects of polypharmacy in the elderly.

ABSTRACT

Objectives@#To study the relationship between health literacy and mild cognitive impairment in the elderly.@*Methods@#A total of 326 senile inpatients were recruited in a tertiary general hospital of Yangzhou by taking the convenience sampling method. The influencing factors of mild cognitive impairment and the relationship between health literacy and mild cognitive impairment were analysed. Health literacy score in elderly patients with mild cognitive impairment and normal cognitive function were (64.05±13.52) and (69.37±9.89) points respectively, and the difference between two groups was statistically significant (t=3.872, P<0.05). In addition, there were statistically significant differences between the two groups in age, education level, marital status, family permanent population, occupational nature, exercise, body mass index and hypertension (t=2.302, χ2=3.479-22.075, Z=22.726, P<0.05 or 0.01). Logistic regression analysis showed that health literacy, education level, age, marital status, body mass index and hypertension were independent factors of mild cognitive impairment in the senile inpatients.@*Conclusions@#Health literacy was an independent factor of mild cognitive impairment in hospitalized elderly patients. Higher health literacy may reduce the risk of mild cognitive impairment.

ABSTRACT

Objective To evaluate the reliability and validity of the Chinese version of the Newest Vital Sign(NVS). Methods The NVS was translated and back-translated. Cultural adaption of scale was performed by Delphi expert consultation and pilot study. The reliability and validity of the Chinese version of the NVS was tested in 451 Chinese residents. Results Chinese version of the NVS consisted of 6 items with Pearson correlation between item and total score of scale ranging from 0.50 to 0.71. Two factors were abstracted by exploratory factor analysis and explained 61.51% of total variance. Confirmatory factor analysis showed that the fitness of the model was acceptable:comparative fit index was 0.96, Tuker-Lewis index was 0.92, standardized root mean square residual was 0.04, root mean square error of approximation was 0.077. Cronbach α coefficient was 0.71, retest reliability was 0.92. Conclusions Chinese version of the NVS has good reliability and validity, which can be used to test the health literacy of residents in China.

ABSTRACT

OBJECTIVE:To observe the clinical efficacy of Shengxuening tablets reducing the risk of microinflammation in maintenance hemodialysis(MHD)patients with renal anemia. METHODS:60 MHD patients were randomly divided into observa-tion group and control group with 30 cases in each group. The total dialysis times of all the patients were 12 h every week at blood flow of 200-250 ml/min and dialysis flow of 500 ml/min. Both groups received routine treatment;observation group was additional-ly given Shengxuening tablets orally 1.0 g,tid;control group was given ferrous succinate 0.2 g,tid, orally;both groups were giv-en subcutaneous injection of erythropoietin (EPO),10 000 IU/time,once a week;treatment for all of the patients lasted for 3 months. The fasting venous blood sample were collected at the beginning of treatment and in the morning every one months. The levels of hypersensitive C-reactive protein(hs-CRP),tumor necrosis factor α(TNF-α),IL-6,hemoglobin(Hb),and hematocrit (Hct)were detected. RESULTS:After 3 months of treatment, total effective rate of 2 groups were 90.00%,the levels of hs-CRP、TNF-α、IL-6 in observation group were much lower than that before treatment and control group after treatment, with statistical sig-nificance(P<0.05 or P<0.01). No severe ADR was found in 2 groups, and the incidence of ADR in observation group(6.67%) was significantly lower than in control group (13.33%),with statistical significance (P<0.05). CONCLUSIONS:Shengxuening tablets have good curative effect, and they can reduce the risk of microinflammation due to the long-term use of EPO and chalybeate.

ABSTRACT

Older people generally had low health literacy, age related cognition decline and some even progress into dementia, burden on society was aggravating. Cognition decline of the elderly could lead to the decrease of health literacy. Meanwhile, higher level of health literacy was a protective factor against cognition decline in the elderly. This paper aimed to explore the relationship between health literacy and cognitive function in the elderly population without dementia, and to provide a theoretical basis for improving the health literacy and cognitive function of the elderly.
